Submit your exemption documents

Businesses claiming Wisconsin sales-tax exemption should submit the documents below. Email destination: qc-jerri@tosatool.com. Jerri reviews submissions and marks the account tax-exempt after approval.

Document checklist

  • Fully completed Wisconsin Form S-211 (or S-211E) Sales and Use Tax Exemption Certificate: download the official fillable PDF and read the instructions.
  • Out-of-state buyers: equivalent state exemption/resale certificate plus Wisconsin seller’s permit number if applicable, or your out-of-state permit number and state.
  • Company legal name, billing/shipping address, and type of business.
  • Reason for exemption (resale, manufacturing, government/educational, etc.).
  • Wisconsin seller’s permit / use tax certificate number, or an allowed alternative such as wholesale only, exempt sales only, or an out-of-state permit.
  • Signed certificate. A signature is required unless submitted electronically under Wisconsin rules.
  • Optional but helpful: W-9 or FEIN letterhead, purchase order referencing exemption, and a contact name, email, and phone.

Important: Uploading documents does not automatically remove tax at checkout. Allow 1–2 business days for review; keep paying/being charged tax until your account is approved and marked tax-exempt.
